Prior to your appointment
Please complete your registration and health history forms from home prior to your first appointment, or arrive at least 30 minutes early to complete them in office.
At your first appointment
Your first appointment will be a consultation, where we will review your dental and medical history, your treatment and sedation options, and address any questions or concerns you may have. If we need additional x-rays, we will take them at this time. We will then schedule the appointment for your oral surgery procedure.
- All patients under the age of 18 will need to be accompanied by a parent or legal guardian.
- A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink from midnight prior to your surgery. You will also need an adult to drive you home.
- Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
Please bring with you
- Your surgical referral slip and x-rays, if available
- A list of medications you're currently taking
- Insurance cards
Insurance Policy
Financial arrangements must be made prior to surgery. Our staff will be very helpful in assisting with insurance providers. At the time of surgery, you will pay “your portion” and we will bill the insurance for the remaining balance.
We accept Medicaid. Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA) may sometimes be utilized as well (specific requirements apply).
Please remember that whether you have insurance or not, you are responsible for the cost of your surgery.
Payment Options
We accept Visa, MasterCard, Discover and American Express. We also offer payment plans through our partner, Care Credit. These arrangements must be set up before arriving for treatment in our clinic.
Cancellations
Please contact our office at least 48 hours in advance for cancellations.

What if my dentist just took x-rays?
If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.
